Living Independently with Support
CLA's Supported Independent Living (SIL) program provides individualized services based on each person's Strengths, Abilities, Needs, Choices, and Desired Outcomes. Our team works with individuals who live in their own homes or apartments, providing the support they need to manage daily life successfully.
The level and type of support varies based on each person's unique situation. Some individuals may need just a few hours of assistance per week, while others benefit from more frequent check-ins and hands-on support.
Areas of Support
Daily Living Skills
Training and assistance with cooking, cleaning, laundry, personal hygiene, and household management.
Medical Coordination
Help scheduling and attending medical appointments, medication management, and health monitoring.
Budgeting & Finances
Support with money management, bill paying, budgeting, and financial planning.
Transportation
Assistance with transportation to appointments, work, shopping, and community activities.
Community Involvement
Support connecting with community resources, social activities, and building meaningful relationships.
Goal Planning
Working together to identify personal goals and develop a plan to achieve them, step by step.
Every SIL plan is built around the individual. We start by understanding what each person wants for their life, then provide the support needed to make it happen. The goal is always to maximize independence while ensuring safety and well-being.
